Between 7–26 out of every 100 people experience shoulder pain at any given time, with some occurring on a daily basis. For many, shoulder pain does not go away quickly: nearly 40 out of 100 people who see a doctor for shoulder pain still have problems a year later.  Half diagnosed with shoulder pain are referred to a physical therapist for treatment but evidence of effectiveness is limited.  Chronic shoulder conditions may even require surgical intervention.

A large proportion of shoulder problems can be attributed to the rotator cuff.  Rotator cuff pain from muscle injury is a common reason people stop doing things they enjoy or find daily life harder. The rotator cuff, located in the back of the shoulder, consists of four muscles that attach to the top of the arm bone via tendons. Together they form a circular/fan-shaped “cuff” that surrounds the head of the upper arm bone (humerus) and blends into the shoulder joint to help the arm rotate in its socket.  When these muscles are strained, overused, irritated, or torn, the shoulder can become weak, stiff, and painful. That pain makes simple tasks — reaching, lifting, dressing, or sleeping — feel painful or impossible.

Everyday activities that are often affected include:

  • Reaching overhead to get dishes, change a light bulb, or put on a jacket.
  • Reaching behind your back to fasten a bra, tuck in a shirt, or retrieve items from a pocket.
  • Lifting groceries, children, or luggage — even modest weights can hurt.
  • Pushing or pulling motions like opening heavy doors, pushing a vacuum, or using a shopping cart.
  • Personal care tasks such as combing hair, brushing teeth, or putting on deodorant.
  • Sleeping on the affected side, which can cause night pain and poor rest.
  • Work tasks that involve repeated arm motions – painting, overhead repairs, and throwing.

In clinical practice, patients with chronic rotator cuff pain may receive a combination of interventions to manage pain and dysfunction. 

Massage is most effective when combined with other treatments, such as rest, exercise, and medication when appropriate.

The Four Rotator Cuff Muscles and What They Do 8

The rotator cuff enables the arm to move in many directions while keeping the top of the arm bone (humerus) centered in the shoulder socket. Because the shoulder moves so much, the rotator cuff is used a lot and can get sore, strained, or injured. The muscles involved include:

Subscapularis:  This muscle sits on the front of the shoulder blade and turns the arm inward (internal rotation).  It helps with tucking in shirts in or reaching across the body. .

Supraspinatus:  This muscle sits on the top of the shoulder blade (scapula).  It starts the motion of lifting the arm out to the side (abduction).  It helps the arm to lift initially before larger muscles take over. 

Teres Minor: This small muscle is situated beside the infraspinatus.  It also turns the arm outward and helps to keep the shoulder stable.

Infraspinatus: Located on the back of the shoulder blade, this muscle turns the arm outward (external rotation).  It helps with throwing and reaching behind the back.

Studies indicate that 30 minutes of medical massage can provide relief. Furthermore, medical massage reduces the need for frequent medication use and problems that can come with it like side effects and dependence.

Potential Causes of Rotator Cuff Injuries 6-7

  • Rotator cuff problems can be caused by a single accident or by wear and tear over time. Here are the most common reasons:
  • Repetitive Overhead Motion – Doing the same overhead movement over and over – like painting , swimming, tennis, or throwing – can strain the tendons.  Tiny tears form and build up until the tendon becomes weak or painful.
  • Age-Related Degeneration – As people get older, tendons get less flexible and receive less blood flow.  This makes them more likely to fray, develop small tears, or break.
  • Sudden Injury – A fall on an outstretched hand, catching yourself, or a strong pull can tear a rotator cuff or tendon.
  • Impingement and Bone Spurs – The space between the top of the arm bone and the acromion (part of the shoulder blade) can get smaller.  Tendons get pinched under the acromion during motion, causing pain and wear.  Bone spurs can form and rub on the tendons.
  • Poor Posture and Muscle Imbalance  – Rounded shoulders or weak shoulder-blade muscles change how the shoulder moves.  The rotator cuff then has to work harder and may become overused.
  • Inflammation (Tendonitis and Bursitis) – Swelling of the tendons and a tiny cushion called the bursa can hurt and make movement painful.

Common Signs and Symptoms of Rotator Cuff Injury 12

  • Pain when lifting the arm or reaching overhead
  • Pain at night, especially when lying on the sore shoulder
  • Weakness in the shoulder or arm
  • A clicking, popping, or grinding sound with movement
  • Trouble reaching behind your back or combing hair
  • Gradual loss of movement

How Medical Massage Therapy Can Help 10-11, 13-16

Tight, sore spots in shoulder muscles — called trigger points — are linked to shoulder pain.  They can develop in many shoulder muscles like the rotator cuff, deltoid, and chest muscles, and are identified by feeling the muscle. Massage to these spots might make the muscle less stiff, help the shoulder move better, and reduce pain.

Medical massage therapy :

  • Reduces muscle tightness: Tight muscles around the shoulder can pull the joint into awkward positions, making pain worse. Gentle kneading warms and relaxes muscles so the shoulder can move more easily.
  • Decreases pain and muscle guarding: When the body experiences pain, nearby muscles tighten to protect it (called guarding).  Soft and deep tissue techniques loosen tighter muscle layers and pressing on trigger points eases knots that refer pain to other areas.
  • Improves blood flow: Compression clears waste products from cells, drains excess fluid from vessels to reduce swelling, and improves delivery of oxygen and nutrients to injured areas, which helps healing.
  • Breaks down scar tissue and adhesions: After injury, stuck or thick tissue can limit motion.  Cross-friction techniques can help soften these areas and remodel healing tissue.
  • Releases myofascia: Gliding pressure along muscle fibers lengthens tissue, reduces adhesions that cause stiffness, and frees tight connective tissue (fascia) around the shoulder.
  • Improves range of motion:  Passive stretching and mild movement of the joint restores motion. This makes exercise and rehab easier.
  • Complements physical therapy: Massage prepares the muscles for stretching and strengthening exercises that rebuild shoulder stability.

Research Findings 17-26

Soft-tissue massage is commonly applied for shoulder pain and some trials suggest short-term improvements in pain, range of motion, and function.  However, the overall evidence is limited by small sample sizes, heterogeneous diagnoses (e.g., mechanical shoulder pain vs. impingement), varied massage techniques and adjunct therapies, short follow-up, and few placebo-controlled comparisons. Consequently, effectiveness remains uncertain: massage may offer short-term symptom relief for some patients, but higher-quality, longer-term randomized trials comparing massage with placebo and other active treatments are needed to define its clinical significance.

When to See a Doctor 27

Some shoulder injuries need imaging, injections, or surgery. Early evaluation helps you get the right care.  Seek immediate medical attention in the event of:

  • Sudden swelling or severe pain after an injury
  • Weakness that limits use of shoulder or arm
  • Pain that worsens or lasts more than a few weeks despite self-care
  • Swelling, redness, or tenderness and warmth around the joint

Shoulder pain accompanied by certain symptoms may be a sign of a heart attack. Seek emergency medical care if you are experiencing:

  • Difficulty breathing or shortness of breath
  • Tightness in your chest
  • Sweating

The rotator cuff is a small but powerful team of four muscles that control shoulder rotation and stabilize the joint. Injuries come from repetitive movements, age, trauma, impingement, and poor posture.
